David C. Johnson (herein after referred to as The Artist) and _______________________ (herein referred to as The Patron), have entered into a binding agreement for the artist to execute a Portrait(s) of ___________________________ (herein referred to as The Sitter) on this day ___/___/___. This Portrait will be done in the agreed upon medium of _________________ on ______________. The pose of the sitter will be: frontal ___, profile ____ or ¾ angle ____. The length of the Portrait will show: the full figure ___, half length____ or bust ____. The background of the Portrait (what is shown behind The Sitter) will be either a neutral background or will render __________________________.

The Portrait(s) will be executed from: life____, photographs taken by the artist ____ or a combination of the two _____.

The Portrait referred to in this Agreement of Portrait Commission document shall be executed in consideration of the agreed upon sum of $______________. Of that sum, a 50% non-refundable deposit is required to initiate the portrait process. Upon final approval the balance of the total sum shall be paid by The Patron in exchange for the Portrait. If after initial presentation of the Portrait, The Patron desires changes to be made; up to three changes shall be made at no additional charge. If those changes do not satisfy The Patron, additional changes will be made at the rate of $75.00 per hour, in addition to the agreed upon sum for commission. If after an additional time has been spent by The Artist on the Portrait to his satisfaction but The Patron is not satisfied, the Commission is null and void, The Patron forfeits his deposit and The Artist holds the right of ownership of the Portrait.
